Before you begin, please note the following:
• You will be prompted to upload your organization's print quality logo. It is recommended that you have this file available before you begin. Accepted file types: 300 dpi JPEG or vector EPS. Logos can also be e-mailed to: UCEAOstaff@gmail.com • Two methods of payment are available: "Pay by credit card" or "Send me an invoice" • After you have completed this form, you will be provided with your sponsorship ID # and a link to the complimentary registration form. The sponsorship ID # and link will also be sent in your confirmation e-mail. • Please complete the complimentary registration form for each person attending the conference (including your complimentary exhibit booth representative). • You will need to enter your sponsorship ID # on all complimentary registration forms. • All complimentary registration forms must be submitted no later than Friday, March 16, 2012.
